Intake forms will be emailed to you prior to your scheduled
appointment following receipt of your appointment confirmation.
All new clients must fill out an intake form in order to be seen.
Existing clients please amend your forms with any
updates concerning your health since your last visit.
We understand that unanticipated events happen occasionally in everyone’s life
but in our desire to be effective and fair to all clients, the following policies are honored.
24 hours advance notice is required when cancelling or rescheduling an appointment
This allows the opportunity for someone else to schedule an appointment in your place.
If you are unable to give 24 hours advance notice you will be charged the full amount for your missed massage service per the discretion of your provider. This amount must be paid prior to your next scheduled appointment.
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show" and will be charged in full for their missed appointment.
Table time begins at the time of your scheduled appointment.
Please arrive early in order to allow for consultation and massage preparation.(5-10 minutes).
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for payment of the time slot you reserved.
